How to make a claim

In order for us to process your claim, we require the following:

1. Claimant's Statement
2. Original Death Certificate
3. Original Policy Document (if lost, to submit Declaration of Loss Policy Form)
4. Obituary details, if any
5. Original NRIC(s) of Claimant(s)
6. Please ensure you have the following documents ready:

Claimant Document Required
Spouse • Original Marriage Certificate
Children • Original Birth Certificate of Children
Administrator • Letter of Administration
Parents • Original Birth Certificate of Deceased
Siblings • Original Birth Certificate of Deceased
• Original Birth Certificate of Siblings
Reason of Death Document Required
Medical Condition • 2 Clinical Abstract Application Forms
• Medical Reports from attending doctor
Accidental Condition • Photocopy of Police Report

We will process your claim upon receipt of all relevant documents and we will contact you should we require additional documents. To help us expedite the processing, kindly submit all relevant documents to us through your servicing agent or mail to:

Important Notes

  1. Original documents must be verified by our Customer Service Representative and will be returned immediately after verification.
  2. Additional medical report fee incurred during the process of the claim is at the expense of the claimant.
  3. HSBC Insurance (Singapore) Pte. Limited does not admit liability by the mere issue of the claim form.
