Group Hospital & Surgical Claim / Individual Hospital & Surgical Claim

In order for us to process your claim, we require the following:

1. Completed Hospital & Surgical Claim Form (Part 1)
2. Original Finalized and Itemized Hospital Bills
3. Doctor's Bills and Receipts
4. Inpatient Discharge Summary/Day Surgery Admission Form (for admission to government/restructured hospitals)
5. Medical Report (Part 2 - for admission to private hospitals/overseas hospitals/clinics)

We will process your claim upon receipt of all relevant documents and we will contact you should we require additional documents. To help us expedite the processing, kindly submit all relevant documents to us through your servicing agent or mail to:

1. Written notice and proof of the claim must be given to us within 30 days from:
   - the date of the death; or
   - the date of discharge from hospitalisation; or
   - the date on which the expenses were incurred for which the claim is made, whichever is applicable.
2. Completing the Hospital Claim Form
   For Government/Restructured Hospitals
   - complete Part 1 of Claim form
   For Private Hospitals/ Clnics/Hospitals outside Singapore
   - complete Part 1 of Claim form
   - complete and sign Part 2 Medical Report Form by attending physician/surgeon
3. We will only consider a claim if:
   - all required documents, evidence and information are provided at the claimant's expense; and
   - all documents, evidence and information provided satisfy our requirements on notice and proof of claim.
4. HSBC Insurance (Singapore) Pte. Limited does not admit liability by the mere issue of the claim form.
