1. Completed Hospital & Surgical Claim Form (Part 1)
2. Original Finalized and Itemized Hospital Bills
3. Doctor's Bills and Receipts
4. Inpatient Discharge Summary/Day Surgery Admission Form (for admission to government/restructured hospitals)
5. Medical Report (Part 2 - for admission to private hospitals/overseas hospitals/clinics)
