Group Outpatient Claim

In order for us to process your claim, we require the following:

1. Completed Outpatient Claim Form
2. Original Bills and Receipts
3. Copy of Referral Letter for Outpatient Specialist Consultation/Diagnostic Test/X-ray Claims)

We will process your claim upon receipt of all relevant documents and we will contact you should we require additional documents. To help us expedite the processing, kindly submit all relevant documents to us through your servicing agent or mail to:

Important Notes

  1. Written notice and proof of the claim must be given to us within 30 days from the date on which the expenses were incurred for which the claim is made.
  2. We will consider a claim if:
    - the Outpatient Specialist Consultation and Diagnostic X-ray and Laboratory Test is referred to by a Medical Practitioner;
    - all required documents, evidence and information are provided at the claimant's own expense; and
    - all documents, evidence and information satisfy our requirements on notice and proof of claim.
  3. HSBC Insurance (Singapore) Pte. Limited does not admit liability by the mere issue of the claim form.
