To make a claim on your policy, please download the claim form and complete all relevant portions. You can help us speed up the processing of your claim by following the instructions on the claim form, which will advise you of the documentation you need to provide to support your claim which includes:

1. Original copies of hospital treatment bill/invoice
2. Copies of medical certificate
3. Clinical report on your medical condition

You must lodge your claim form with us not later than 14 days from the date of your injury or 60 days from the date of your hospital confinement.
