To make a claim on your policy, please download the claim form and complete all relevant portions. You can help us speed up the processing of your claim by following the instructions on the claim form, which will advise you of the documentation you need to provide to support your claim which includes:
1. All medical and clinical report on your medical condition
2. Police report and incident report if your medical condition arises from an accident
3. Death Certificate (in respect of death claim)

You must lodge your claim form with us not later than 60 days from the date of first diagnosis of your Critical Illness as defined in the policy.
