In order for us to process your claim, we require the following:
- Critical Illness Claim Form (Duly completed and signed by Claimant)
- 2 Clinical Abstract Application Forms
- Relevant laboratory reports (ie. histopathology report, biopsy report, CT-scan results, ECG results, etc.)
- Copy of National Registration Identity Card (NRIC) / Identification document of Claimant
- Medical Reports from attending doctor(s) (please use the appropriate form based on the critical illness which you are claiming)
- Alzheimer's disease
- Apallic syndrome
- Aplastic anaemia
- Bacterial meningitis
- Benign brain tumour
- Blindness (loss of sight)
- Cancer
- Chronic liver disease
- Chronic lung disease
- Coma
- Coronary artery surgery
- Deafness (loss of hearing)
- Elephantiasis
- Encephalitis
- Fulminant viral hepatitis
- Heart attack
- Heart valve surgery
- HIV due to blood transfusion
- IDDM
- Kawasaki disease
- Loss of speech
- Major burns
- Major head trauma
- Major organ or bone marrow transplant
- Medullary cystic disease
- Motor neurone disease
- Multiple sclerosis
- Muscular dystrophy
- Necrotising fasciitis
- Occupationally acquired HIV

How to make a claim
We will process your claim upon receipt of all relevant documents and we will contact you should we require additional documents. To help us expedite the processing, kindly submit all relevant documents to us through your servicing agent or mail to:
